School Entry Requirements 

Sports Physical: Any student who will be in grades 7-12 for the 2022-2023 school year, and wishes to participate in extra curricular athletics must have a current sports physical done after May 1, 2022 as directed by the Kansas State High School Activities Association.

The annual sports physical clinic will be held on Wednesday, May 4, 2022 at Memorial Health System’s Heartland Healthcare Clinic from 6:00 pm to 7:00 pm.  

 Who: Current USD #435 students who will be in grades 7-12 in the 2022-23 school year who would like to participate in school sports next year.

What:  Annual Sports Physical Clinic

Where:  Memorial Hospital—Heartland Healthcare Clinic

When:  Wednesday, May 4, 2022

Cost:  $10  Cash only.  No checks, no debit/credit cards, no insurance will be filed

Time:  6-7 pm

*Everyone will be required to wear a mask while inside Memorial Health System to follow their guidelines.  Each person will need to provide their own mask.  

*Paperwork may be downloaded from the school nurse page on the school website, and will be available in the AHS & AMS offices after April 27,2022. 

*PHYSICALS COMPLETED PRIOR TO MAY 1, 2022 ARE NOT VALID FOR THE 2022-23 SCHOOL YEAR—PLEASE DO NOT get a physical prior to May 1, 2022 for next year’s athletics.  
 
*Packets must be completed and all parent and student signatures must be in place prior to getting in line.

*Only students will be permitted inside the building for the physical.  Parents/guardians may not accompany their student inside.  

 We strongly encourage all students who may be considering participating in school athletics next year to take advantage of this program.  All students who participate MUST have a valid sports physical and required forms dated after May 1, 2022 (using the new KSHSAA form dated 3/2020—old forms cannot be accepted).  These must be on file with the school prior to being allowed to practice for the 2022-23 school year.  
 
Physician offices are very busy, and it can be very difficult to get an appointment prior to practices beginning.  This means your student will have to sit out until an appointment can be completed.  

Immunization record: Please bring a copy of your child's immunization record with you when enrolling, if you are new to the district or have had any updates.


Administer Medicine: All medicines distributed at an Abilene School must have a Request to Administer Medicine form on file.

School Entry Physicial: All students between the ages 5-8 who is enrolling in a Kansas school for the first time must also present a completed physical examination that has been done in the previous 12 month period.

Food Allergy/ ModificationAny child that has a food allergy/intolerance or other dietary needs, will need to have a dietary form on file.  This form needs to be filled outeach year and signed by the child's physician.
