School Entry Requirements

Sports Physical:  Any student who will be in grades 7-12 for the 2011-12 school year, and wishes to participate in extracurricular athletics must have a current sports physical done after May 1, 2011 as directed by the Kansas State High School Activities Association.


      

                                    

Immunization record:  please bring a copy with you when enrolling, if you are new to the district.  There are some changes for the 2011-2012 school year in immunization requirements.  Please note that Tetanus booster ages have changed, and that there are some new requirements. 


Diphtheria, Tetanus, Pertussis (DTaP): five doses required.  Four doses     acceptable if dose 4 given on or after the 4th birthday. 


Tdap booster required in 7th grade. A booster is also required if the student has previously received a only a Td booster (no pertussis added) and it has been longer than 2 years ago.


Poliomyelitis (IPV/OPV): four doses required.  Three doses acceptable if dose 3 given on or after the 4th birthday.


Measles, Mumps, Rubella (MMR): two doses required.


Hepatitis B: three doses required through grade 11.


Varicella (chickenpox): two doses required for Kindergarten, 1st, 2nd, and 7th grades; one dose required grades 3-6 and 8-11 unless history of varicella disease documented by a licensed physician is provided to the school. Two doses are currently recommended by the ACIP for all ages.


Administer Medicine: All medicines distributed at an Abilene School must have a Request to Administer Medicine form on file.

School Entry Physical: Any student between the ages of 5-8 who is enrolling in a Kansas school for the first time must also present a completed physical examination that has been done in the previous 12 month period.

Food Intolerance/Allergies: Any child that has a food allergy/intolerance or other dietary needs, will need to have a dietary form on file.  This form needs to be filled out each year and signed by the child’s physician.
